POLH基因编码DNA聚合酶η(poleta),是一种重要的DNA修复酶,负责在紫外线照射等DNA损伤情况下进行跨损伤合成(translesion synthesis, TLS)。在跨损伤合成过程中,poleta能够绕过DNA损伤位点,在损伤位点对面引入核苷酸,从而维持基因组的稳定性。POLH基因的突变会导致poleta的活性降低,使DNA损伤无法得到有效修复,从而增加癌症的风险,如皮肤癌和黑色素瘤等[1,2,6]。

在皮肤癌患者中,POLH基因的突变并不是导致皮肤癌的主要因素。研究发现,在40名皮肤癌患者中,POLH基因的编码序列并未发生突变,且POLH mRNA在所有正常和皮肤肿瘤组织中均有表达[1]。这表明,POLH基因的突变并不是皮肤癌发生的主要原因,而可能是POLH基因的表达水平或表达方式的变化导致了皮肤癌的发生。进一步研究发现,POLH基因的表达水平在皮肤癌和正常组织之间存在差异,其中1/7的鳞状细胞癌(SCC)和4/10的基底细胞癌(BCC)肿瘤组织中POLH表达水平降低,而1/7的SCC和3/10的BCC肿瘤组织中POLH表达水平升高[1]。这表明,POLH基因的表达水平变化可能是皮肤癌发生的一个重要因素。

POLH基因突变是导致Xeroderma Pigmentosum Variant(XPV)的主要原因。XPV是一种罕见的常染色体隐性遗传病,患者对阳光中的紫外线敏感,容易发生皮肤癌。POLH基因编码的poleta在DNA修复过程中发挥重要作用,其突变会导致poleta活性降低,使DNA损伤无法得到有效修复,从而增加皮肤癌的风险[2,7]。研究发现,在两个有亲缘关系的巴基斯坦家族中,POLH基因发生了移码突变,导致poleta蛋白的截断和功能丧失,从而导致了XPV的发生[2]。

POLH基因的表达水平变化不仅与皮肤癌的发生有关,还与肝细胞癌(HCC)的预后相关。研究发现,POLH-AS1在HCC组织和细胞中表达水平升高,且POLH-AS1的表达水平与HCC患者的生存期和预后相关。进一步研究发现,POLH-AS1的表达水平升高与HCC的进展和肿瘤的免疫微环境相关[3]。

POLH基因的表达水平变化还与Ig基因超突变有关。研究发现,在REV1催化活性缺陷的POLH基因敲除小鼠中,Ig基因超突变的频率显著降低,且C到G和G到C的颠换突变几乎消失[4]。这表明,POLH基因在Ig基因超突变过程中发挥重要作用,且可能与REV1基因存在遗传相互作用。

POLH基因的表达水平变化还与杆状病毒的polh基因表达有关。研究发现,在AcMNPV 25K基因突变的病毒中,polh基因的表达水平降低,且25K基因产物对polh基因的转录具有重要作用[5]。
